KEY DOCUMENTS​​​
To adequately evaluate and prepare your bankruptcy, please provide the following key documents:

Last 2 years of personal State and Federal income tax returns with W-2s.

Last 2 years of State and Federal income tax returns for any business that you own.

VIN numbers, color, and mileage for any vehicles titled in you or your spouses names.

Copy of you and your spouse's drivers licenses and Social Security cards.

If you own a home, provide verification when purchase occurred and if any of your debts preceded your buying it.

Your last three months of bank statements as well as the month that you filed bankruptcy.

If you own a business I need to know the value of the business and any debt that the business owes.

The most recent billing or correspondence for any debt you owe (provide account number, creditor name, address, and balance). Please also provide a copy of the most recent bill.

Proof of you and your spouse's income for the last six months (income is based upon the six months prior to filing) leading up to the last day of the month before you filed. If you are self employed, provide a monthly income and expense report for each of those six months and ongoing until we file

If you have been divorced, provide a copy of the Decree and Stipulation setting out what debts you are responsible for under the Order and what you are to pay or receive for spousal and child support.

A list of all assets with an approximate value of the assets. I can provide a form to fill out; however, primary assets of interest include real estate, motor vehicles, boats, ATV's, firearms, stocks and bonds, retirement accounts, and bank accounts.

Addresses where you lived for the past three years and when you lived there.

If you receive child support, spousal support, social security benefits or Veteran's Disability benefits I need to know how much you receive each month, how much you have received this year, and how much you have received for the past two years.

If you have sold or given away anything over the last two years, I need to know what you sold/gave away, what each item was valued at, when each instance occurred, who you sold/gave away each item to, and how much you received in return.
